USMLE Step 1 - United States Medical Licensing Examination Practice Test

As of January 26, 2022, USMLE Step 1 is reported as Pass/Fail only. The exam format consists of approximately 280 multiple-choice questions, divided into seven 60-minute blocks, administered in a single 8-hour testing session. The content integrates basic science principles across disciplines, presented primarily in the form of clinical vignettes. While a numeric score is no longer provided, the passing standard remains rigorous and is set by the USMLE program.

Eligibility is typically granted to medical students and graduates from: 1) LCME-accredited U.S. or Canadian medical schools, 2) AOA-accredited U.S. osteopathic medical schools, or 3) international medical schools listed in the World Directory of Medical Schools. Students are usually required to have completed, or be near completion of, the basic medical sciences curriculum at their institution. Specific eligibility steps must be completed through the appropriate registration entity (NBME, ECFMG, or FSMB).

The shift to Pass/Fail has fundamentally altered the residency application landscape. While it reduces the pressure associated with a specific three-digit score, it elevates the importance of other application components. Residency program directors now place greater emphasis on USMLE Step 2 CK scores, medical school performance (grades, class rank), letters of recommendation, research experiences, clinical clerkship evaluations, and personal attributes. The change aims to promote holistic review but necessitates a broader and more well-rounded preparation of the applicant's entire portfolio.

Step 1 is an integrated exam focusing on the application of basic science to medicine. The major content areas, as defined by the USMLE content blueprint, include: Anatomy, Behavioral Sciences, Biochemistry and Molecular Biology, Microbiology and Immunology, Pathology, Pharmacology, and Physiology. Interdisciplinary topics such as Genetics, Aging, Nutrition, and Ethics are woven throughout. Questions frequently require synthesizing knowledge from multiple disciplines to solve a clinical problem.

Most U.S. medical students take Step 1 after their second year, following completion of core basic science courses. Dedicated preparation periods typically range from 4-8 weeks of full-time study. A successful strategy involves an initial content review using trusted resources (e.g., First Aid for the USMLE Step 1, Pathoma, SketchyMedical), followed by extensive practice with question banks (e.g., UWorld) to apply knowledge and build test-taking endurance. Self-assessments like the NBME Comprehensive Basic Science Self-Assessments and the USMLE Free 120 are critical for gauging readiness. Creating a structured daily schedule that balances content review, question blocks, and analysis of incorrect answers is essential.
