ABIM Internal Medicine Board Exam Practice Test

The exam is a single-day, computer-based test administered at Pearson VUE testing centers. It consists of approximately 240 single-best-answer, multiple-choice questions, primarily presented as patient-centered vignettes. The exam is divided into four sessions of 60 questions each, with a total testing time of 10 hours, which includes allotted break time. Questions assess medical knowledge, diagnostic reasoning, and clinical judgment across the entire spectrum of internal medicine.

Candidates must have graduated from an accredited medical school, completed an ACGME-accredited (or equivalent) internal medicine residency training program in the United States or Canada, and possess a valid, unrestricted medical license. Documentation of training completion, including verification of clinical competence by the program director, is required. The ABIM must deem an applicant's credentials satisfactory before permitting registration for the exam.

The ABIM uses a criterion-referenced passing standard, meaning your performance is measured against a predefined level of knowledge and skill, not against other test-takers. The score is reported on a scale of 200 to 800, with a passing score of 366. You will receive a "Pass" or "Fail" result, along with a confidential score report that provides feedback on performance in major content categories relative to the passing standard.

The ABIM MOC program is a continuous process designed to ensure diplomates remain current in medical knowledge and practice. It consists of four requirements: 1) Professional Standing (maintaining an active, unrestricted license), 2) Lifelong Learning & Self-Assessment (earning CME credits and completing ABIM-approved self-assessment activities), 3) Assessment of Knowledge, Judgment & Skills (passing a secure exam every 10 years or participating in the longitudinal knowledge assessment option), and 4) Improvement in Medical Practice (engaging in practice-based quality improvement activities). Meeting these requirements is necessary to maintain 'Certified' status.

Successful preparation involves a multi-faceted approach: a comprehensive review of core internal medicine textbooks or dedicated board review books, completion of extensive question banks (e.g., MKSAP, UWorld, MedStudy) to familiarize yourself with the question style and identify knowledge gaps, and participation in a board review course, either live or recorded. Creating a structured study schedule months in advance, focusing on weak areas identified through practice tests, and dedicating time to review ethics, patient safety, and biostatistics are critical strategies. The most effective study integrates active learning through question practice with systematic content review.
