BCPS Board Certified Pharmacotherapy Specialist Exam Practice Test

Candidates must hold a current, active license to practice pharmacy in the U.S. or another jurisdiction. Additionally, they must complete one of two pathways: 1) Four years of practice experience with at least 50% of time spent in pharmacotherapy activities as defined by BPS, OR 2) Successful completion of a PGY1 pharmacy residency accredited by ASHP. The BCPS exam is a computer-based test consisting of 200 multiple-choice questions, of which 175 are scored. Candidates are given 4 hours to complete the exam. Questions are designed to assess application and analysis, not just recall, and are based on the official exam content blueprint. The blueprint outlines the percentage of questions from each domain, such as Cardiology, Infectious Diseases, Neurology/Psychiatry, and System-Specific and Patient-Specific Pharmacotherapy.

BCPS certification is valid for seven years. To recertify, specialists must either pass the recertification examination or complete a structured professional development program (which includes continuing education and practice assessment activities) and accumulate 100 units of recertification credit through the BPS Professional Development Program (PDP). This process ensures certified specialists maintain contemporary knowledge and skills in pharmacotherapy.

BCPS is a broad, foundational specialty certification focused on comprehensive medication management across all disease states and patient care settings. Other BPS certifications (e.g., Cardiology, Infectious Diseases, Oncology) are focused on a specific, narrower therapeutic area. BCPS is often pursued first as a demonstration of broad clinical expertise, upon which additional subspecialty certifications can be added. It is the most widely held pharmacy specialty certification.

BCPS-certified pharmacists are highly sought after for roles as clinical pharmacy specialists, clinical coordinators, pharmacy clinical managers, and faculty at colleges of pharmacy. They are integral members of interdisciplinary teams in hospitals, health systems, ambulatory care clinics, managed care organizations, and within the pharmaceutical industry in medical affairs or drug development roles. The credential is frequently required or preferred for these advanced positions.
