NBOME COMLEX-USA Level 1 Practice Test

The primary purpose is twofold: first, to assess a candidate's mastery of the foundational biomedical and clinical sciences necessary for safe and effective patient care, and second, to serve as a mandatory step toward osteopathic medical licensure in the United States. It ensures all osteopathic physicians meet a nationally standardized level of competency before advancing to clinical training.

OPP is not tested in isolation but is fully integrated throughout the exam. Questions will present clinical vignettes where understanding somatic dysfunction, osteopathic manipulative treatment (OMT) indications/contraindications, and the body's self-regulatory mechanisms are essential for diagnosis and management. This tests the application of osteopathic philosophy in the context of general medical knowledge.

While both exams cover similar foundational biomedical sciences, COMLEX-USA Level 1 is specifically designed for osteopathic medical students and includes significant, integrated content on Osteopathic Principles and Practice (OPP). Additionally, COMLEX-USA utilizes a unique question style that often emphasizes clinical presentation and patient-centered care from an osteopathic perspective. It is the required pathway for DO licensure.

The NBOME sets a minimum passing score that is reviewed periodically. COMLEX-USA Level 1 is scored on a three-digit scale.
